letter for whatever is missing (certification, extra training, fingerprint check, etc.)

                          STEP BY STEP

How to meet the Florida Mold Licensing Law Requirements before March 1, 2011--READ CAREFULLY

Go to: https://www.myfloridalicense.com/intentions2.asp?chboard=true&boardid=07&SID= and download the application from the FL Mold Related Services Board either the Mold Assessor - Initial License by Grandfathering or Mold Remediator - Initial License by Grandfathering

Complete the application and, on the first page, choose whether you want ACTIVE or INACTIVE status (Inactive status waives the insurance requirement but, if you decide later to activate, will require you to make-up your CEUs, seven hours per year, for each year you remain inactive)

Here how you get the documentation you need:

1) Exam Results--when you took your proctored examination, you received an email letter with your grade.  Print that out.
2) Membership certification--go to www.NORMIPro.com and click on "verify member status" (http://www.normipro.com/verifycert.php).  Type in your last name and print this page.  It will indicate that you are ACTIVE and the expiration date of your certification.  If you have not yet activated your membership, go to www.Join.NORMI.org and do so as the state will require you to be active with a nationally recognized organization.
3) At least 60 hours of education on mold assessment--you received a verification sheet of 32 hours on the last day of your class evidencing 32 hours and the CEUs available for different organizations.  If you cannot find that, email support@normi.org for a copy--be sure to identify the class date and location.
4) As proof of completing 60 hours of education on mold assessment please submit:
5) Proof of completion that indicate course type and hours earned--you will have to prove the additional 28 hours of training.  One way to do that would be to take the BTS CMA Bundled Class online (http://www.besttrainingschool.com/bundle.php).  At this website you will need to create an account with a unique username/password and purchase the "Bundled CMA Package".  You will have 30 days to complete the course unless you also include the BTS Enrollment Subscription which provides ongoing access to all the classes you have or will take online.  Once you have completed the course, go to the BTS Test Center ("Test Center" link on your Membership Account Information page).  Once you've completed the course, "CMA Bundle Quiz" test using the password.  Once you have completed the quiz you will be given the option to print out a document evidencing the 28 hours and your passing grade.  That's it.

Mold Assessor: Have you obtained general liability and errors and omission for preliminary and post remediation mold assessment insurance coverage in the amounts of $1,000,000 each  YES or NO --if you have not secured insurance, please contact Rod Morton with IOA for a quote...his information is in your member section of the website and is for ACTIVE members only!

1) Complete sections I, II, III, VII, VIII, IX of this application
2) Pay $330 fee (this is a two year license)
3) Complete electronic fingerprint background check--this is handled by Pearson Vue (www.PearsonVue.com) and they have several locations throughout the state.  If you DO NOT live in the State of Florida, call the Florida DBPR (850.487.1395) and request that they send you the fingerprint forms.  You can then have them taken locally (usually the local police department).  Then send the fingerprints to Pearson Vue, 3131 South Vaughn Way, Suite 205, Aurora, CO 80014 along with $57.25 payable to Pearson Vue BY MONEY ORDER ONLY and they will forward the information to the DBPR.
4) Submit proof of certification as a mold assessor by a state, or national association that requires passing a proctored examination--this is a copy of your NORMI CMA Certificate.  To get that you must have done the following:
     a) Taken the 32 hour CMA/CMR class,
     b) Faxed a copy of your resume to 866.211.4324
     c) Faxed a copy of your Certificate of Insurance to 866.211.4324 endorsed to NORMI, 22174 Prats Road, Abita Springs, LA  70420
     d) Passed the proctored examination (CANNOT BE THE ONLINE TEST) with 70% or better grade
     e) Activated your NORMI Membership (www.Join.NORMI.org), and
     f)  Paid the associated certification fees (if you haven't paid them, email support@normi.org for directions on how to do so)
5)  At least 60 hours of education on mold assessment--this is just a reminder of what you've already prepared from Page #10, items #3 and the same document referred to on Page #10, items #3 and #5

Be sure everything is signed, make a copy and then mail the entire application WITH your check and documentation to Department of Business and Professional Regulation, 1940 North Monroe Street, Tallahassee, FL  32399-0783
